     Documenting Certain Charitable Contributions:  Contribution of $250 or more requires contemporaneous written substantiation from the Charity. Jopa Ministries, Inc. will provide, at the request of the donor, a computer-generated form providing sufficient information to substantiate the amount of the deductible contribution made by the donor.

     Also, Jopa Ministries, Inc. will furnish disclosure statements to donors for quid pro quo donations in excess of $75 for services consisting entirely of religious and therapeutic benefits such as workshops, seminars and individual, couple and family counseling. The statement will be given only in connection with a donation.
